Client hereby acknowledges and agrees that the Consultant is not nor is holding herself out to be a doctor/physician, nurse, physician’s assistant, advance practice nurse, registered dietician, licensed nutritionist or any other medical professional (“Medical Provider”), or a psychiatrist, psychologist, therapist, counselor, or social worker (“Mental Health Provider”). As a child sleep consultant, Consultant is not providing health care, medical or nutritional therapy services, or attempting to diagnose, treat, prevent or cure any physical, mental or emotional issue, disease or condition; instead, Client hereby agrees that Consultant is solely providing an educational service to Client and that no medical diagnosis or advice is being given by the Consultant. The information provided by the Consultant pertaining to you and/or your child’s sleep, health, or any other aspect of your life is not intended or implied to be a substitute for the professional medical advice, diagnosis or treatment provided by your own Medical Provider or Mental Health Provider. Client further agrees to seek the advice of Client’s own Medical Provider and/or Mental Health Provider regarding any questions or concerns you have about your own health or your child’s specific health or any medications, herbs or supplements you or your child are currently taking or will in the future take.
